Never received benefits my entire leave - or help with them

I used my short term disability insurance for maternity leave and started the process beforehand knowing when I would be out (scheduled induction.) It took a little over a week into my leave to get the forms, then another week to get forms from my doctor to them.
Waiting two more weeks I reached out and asked about the status to which they said it was in review and my they would let me know what my adjuster said. They never responded.

Another week and I called them and was told I was missing 2 forms. When they sent them to me I checked and I had in fact already sent them. I asked to speak to a supervisor and was told they’d fill out that request and they respond in 24-48 hours. I never heard anything.
So I called back after a few days and was told it was approved and a paper check was sent (which will take a week to get here) even though I filled out direct deposit information with my claim.

I also was told it was only for 6 weeks because I didn’t have anything stating otherwise. The forms my doctor filled out said 8 weeks but they said they didn’t have that. But they would put in ANOTHER request for someone to contact me about that issue, which would take a week. By the time they get back to me, if they even do, will have been after 6 weeks so I guess I’ll be back to work possibly. In the time I’ve waited for benefits I haven’t been able to pay my rent, car payment and will have lost daycare because I can’t pay. So I will most likely be unable to return to work anyway with nothing to pay daycare costs with.
