Denied short term disability and need to file for long term with same insurer

Hi, I have long haul Covid. I have been on FMLA leave from work since April 25, 2023. I was denied by my short term disability carrier (Reliance/Matrix) so I need to appeal that. I also need to apply for my long term disability that is with the same insurer because my leave ends in July and I'm not able to return to work. I don't want to mess up my appeal since I only get one and I don't know how to apply for long term and have it be accepted if they denied me for short term. Can you help with this?

As you are likely aware, Long Term Disability is predicated on receipt of Short Term Disability (the STD is the Elimination Period for the LTD period) so in appealing the STD claim, if you are successful there will be a rollover review to determine rights to LTD benefits.
