What does Unum mean by "treatment provider"?

I have been receiving long term disability benefits from UNUM for a neurologic problem and need to complete the Individual Disability Status Update form. I have only one doctor who is treating me for my condition. On the form is a section identified as Information About Physicians. Under this section are the following statements, “Please provide the following about any treatment providers you have seen since you last updated this information. If you are being treated by more than two providers on a regular basis, please share the following information on a separate sheet of paper and include it with this form”. UNUM is asking for the Provider Name, Address, Speciality, Medical Condition, Date of Last Visit and Date of Next Visit for this condition. I am a bit confused as to what is meant by a “treatment provider”. For this section, do I only list the one physician who is specifically treating me for my neurologic problem? Or do I list any physician I have seen such as my dermatologist, gastroenterologist, eye doctor, etc.? Thank you.

Evan, they are referring to the doctors that are treating you for your disabling condition.
