I'm going bankrupt after Hartford rejected all my medical tests/records

In January, I collapsed in a cross walk while crossing the street. I sustained injuries to my left knee and spine and have been chronic testing to determine the cause of my fall. I was admitted to the hospital via ER and discharged over the weekend. That immediate Monday, my PCP submitted a physician statement to The Hartford for a disability claim on 1/20/20. The Hartford has rejected all of my various doctor records and tests. The MRI of my left knee revealed a partial tear of my MCL. Heart studies reveal erratic BP and heart rates that are abnormal. The CT scan revealed a tumor on my ovary. The Hartford has rejected every test and record. I call their Corporate Affairs number that I was provided by the Customer Care team. I’m always told that I will get a returned call,, and I never get a returned call. They’re damaging me because I cannot return to work with a pending surgery to remove an ovarian tumor on 3/24 and I still don’t have treatment for my heart or knee. I’m a consultant and my employer demands that I return at 100%. They will not let me return without a letter stating I’m fit to return but my dr can’t write that because I’m not fit. I have no income. I’m a single handicapped female with no other source of income. I maxed my credit cards. I have no income. I’m going bankrupt. Can you please help?
