Do I have to sign the overly broad "AUTHORIZATION TO OBTAIN AND DISCLOSE INFORMATION" form from The Hartford LTD insurance company?

Claim paid by Hartford for 10+ years. Now they are trying to cut my claim because ALL Doctors now will not fill out ATTENDING PHYSICIAN STATEMENT and I cannot find any Doctors to do any of these tests: Functional Capacity Evaluation, Residual Functional Capacity or Fit For Function. The entire medical community has hung me out to dry. I have contacted hundreds of Doctors, Practices and even the Hospitals' Management Organizations!!!

You have a duty to provide your medical evidence in support of disability to Hartford. You need a doctor to sign your APS form or your benefits will not be paid. If your policy doesn’t require you to sign an authorization then you don’t have to.
