What do I do if my attending physician's office tells me it is their office policy to not fill out Attending Physician Statements for any patient for LTD benefits?

I've had my disability claim approved, but my family physician won't fill out an Attending Statement since he is only managing general medical issues and the claim is centered on cervical spine and pain issues. However, both the spine surgeon and pain management physician's offices say it is their office policy to not fill out LTD forms for any patient regardless of circumstances. My insurance company (Northwest Mutual) sent an Attending Physician statement for Continuing Benefits form at 6 months but I have no doctor willing to fill it out. Do I tell the insurance company I tried but none of my doctors will cooperate or look for new doctors (which looks like I'm doctor-shopping for benefits)?

This is a big problem. You need to find a treating doctor that will complete paperwork. Northwestern Mutual requires completion of the attending physician statement, but they may accept your medical records if they document your restrictions and limitations. We highly recommend you find a new doctor that will cooperate.
