Orthopedic surgeon refuses to complete physicians statement form for my long term disability.
I recently had a total knee replacement surgery on Jan 2023, and have been struggling with tightness, stiffness and pain still and not able to work since June 2022. I was also approved for Social security disability in Sept.2023. My LTD is requesting Work note and Physicians form paperwork and my surgeon that has been seeing me is refusing to complete stating she doesn't do disability. I'm also dealing with stress and anxiety because of my chronic pain and dealing with doctors and getting forms completed. Thank you !

Diana: This is a common problem with treatment providers. There are some steps you can take, including having your primary care provider (PCP) complete the Attending Physician’s Statement.
