What do I do if not necessarily the insurance company denied my claim but employer?

From what I can gather, it sounds like your short term disability policy is self-funded by your employer and the re-couped the STD benefits they paid during the denial from the check you received after returning to work. However, you indicated that your claim was approved after you returned to work so there might be a disconnect with HR regarding the benefits they think they wrongfully paid during the extension that was initially denied. If you were provided an approval letter for that period of time I would send that to HR (or whoever is responsible for payroll) to get the matter rectified.
